In nearly half of all couples struggling to conceive, a male factor is part of the problem. In roughly 20–30% of cases, it is the primary cause. Yet male fertility receives a fraction of the attention, research funding, and clinical focus that female fertility does. Most fertility clinic websites lead with images of women. Most supplement marketing targets women. And in many couples, the man doesn’t get tested until months or years into the process.

This is a clinical and cultural blind spot that costs couples time, money, and emotional energy. A semen analysis takes 30 minutes and costs $100 to $300. It should be the first test ordered—not the last.

What Male Factor Infertility Actually Means

Male factor infertility encompasses any sperm-related issue that reduces the probability of natural conception. The three core parameters are count (how many sperm per milliliter), motility (what percentage are swimming effectively), and morphology (what percentage have normal shape). Abnormalities in any of these can impair fertility.

The World Health Organization defines a normal semen analysis as at least 15 million sperm per milliliter, at least 40% total motility (32% progressive motility), and at least 4% normal morphology. These are minimum thresholds, not optimal values—many men with values above these cutoffs still experience fertility challenges.

Common Causes

Varicocele is the most common identifiable cause, present in approximately 40% of men with infertility. It is an enlargement of the veins within the scrotum that raises testicular temperature and impairs sperm production. Varicocele repair (varicocelectomy) can improve sperm parameters in some men, though the effect on live birth rates is debated.

Hormonal imbalances—low testosterone, elevated FSH, thyroid dysfunction, or elevated prolactin—can suppress sperm production. These are diagnosable with bloodwork and often treatable with medication.

Lifestyle factors play a larger role than most men realize. Obesity, smoking, excessive alcohol, cannabis use, anabolic steroid use (even years prior), excessive heat exposure (hot tubs, laptops on laps, tight underwear), and certain medications (SSRIs, testosterone supplementation, finasteride) can all impair sperm quality. Many of these are modifiable.

Genetic factors including Y-chromosome microdeletions, Klinefelter syndrome, and CFTR gene mutations (associated with absence of the vas deferens) can cause severe male factor that may require IVF with ICSI or donor sperm.

Unexplained male factor accounts for a significant portion of cases where the semen analysis is abnormal but no specific cause can be identified. Sperm DNA fragmentation—damage to the genetic material inside sperm cells—is increasingly recognized as a contributing factor that standard semen analysis does not detect.

Testing: What to Get and When

Every male partner in a couple experiencing infertility should have a semen analysis as one of the first diagnostic steps—not after six months of female testing. The test is non-invasive, inexpensive, and provides information that fundamentally shapes the treatment plan. If the first analysis is abnormal, a repeat test 2–4 weeks later is recommended, as sperm parameters can vary naturally between samples.

If the semen analysis is abnormal, additional testing may include a hormone panel (FSH, LH, testosterone, prolactin, TSH), a scrotal ultrasound (to check for varicocele), genetic testing (karyotype, Y-chromosome microdeletion analysis, CFTR screening), and sperm DNA fragmentation testing ($200–$400, not routinely ordered but increasingly valuable). An at-home sperm test can provide a preliminary count and motility screen, but it cannot replace a full clinical semen analysis.

Treatment Options

Lifestyle modifications are the first line of treatment for mild male factor. Losing weight (a BMI reduction of even 5–10% can improve parameters), stopping smoking and cannabis, reducing alcohol to moderate levels, avoiding heat exposure, switching from tight to loose-fitting underwear, and discontinuing any testosterone supplementation (which paradoxically suppresses natural sperm production) can all improve sperm quality over a 3-month window—the time it takes for a new generation of sperm to develop.

Supplements with the strongest evidence include CoQ10 (200–600mg daily), zinc (30mg daily), L-carnitine (1,000–2,000mg daily), selenium (55–200mcg daily), and vitamin C (500–1,000mg daily). Medication may be appropriate for hormonal causes. Clomiphene citrate (off-label for men) can boost testosterone and sperm production in men with low gonadotropins. Anastrozole may help men with elevated estrogen levels. These should be prescribed and monitored by a reproductive urologist.

Surgical intervention (varicocelectomy) may improve parameters in men with clinical varicocele. The surgery is outpatient with a short recovery, but improvement in sperm parameters typically takes 3–6 months.

IVF with ICSI (intracytoplasmic sperm injection) is the treatment of choice for severe male factor. ICSI bypasses all natural sperm transport barriers by injecting a single sperm directly into an egg. It can achieve fertilization with as few as one viable sperm per egg retrieved. For men with no sperm in the ejaculate (azoospermia), surgical sperm extraction (TESE or micro-TESE) can retrieve sperm directly from the testes for use with ICSI.

The Emotional Dimension

Male infertility carries a particular stigma that female infertility, while painful, does not face in quite the same way. Masculinity is culturally entangled with reproductive capability in ways that make a diagnosis of male factor feel like a judgment on identity, not just a medical finding.

Many men report feeling shame, guilt (toward their partner), and reluctance to discuss their diagnosis—even with close friends or family. Some avoid testing altogether because the possibility of an abnormal result feels too threatening. This avoidance delays diagnosis and treatment, costing the couple time.
